Registration
Player and Coach Registration All Players and Coaches must register via Cybersport or on a USA Hockey IMR Form. Players Born in 2001 and before Players born in 2002 and After Coaches A Background Check form must be completed completed form and given to WBLAHA Registrar, Darlene Turner. Upon receipt, Darlene will register all coaches with USA Hockey. E-mail Darlene at registrar@wblhockey.com with any questions.
The completed form can be e-mailed to Darlene Turner, WBLAHA Registrar, at registrar@wblhockey.com.
They can also be printed but, if you choose to print rather than e-mail the form, it you MUST have it with you in order to register with the WBLAHA.

Please bring insurance information and your skater to sign forms.
Forms (Consent to Treat) can be filled out in advance by downloading them from the Forms section (click within left navigation or on the links above).New skaters to the association should also bring a photocopy of their certified birth certificate and current health insurance information.
All skaters registering must be five years old before July 1, 2008.
